I acknowledge that there is a possible risk for injury (including death) during physical activity and agree to waive any liability from Active Health Solutions, LLC and its employees. This assumption of risk and waiver of liability applies to my immediate family members, non-immediate family members, successors, and heirs. I understand that I am solely responsible for any medical bills as a result of injuries that occurred during any use of the Active Health Solutions, LLC facilities. If I have any concerns about my health or medical status I will consult with my doctor before using the facilities.

I understand that Active Health Solutions is an unsupervised fitness facility and that outside of stated staff hours there will not be anyone to help me use the exercise equipment.  I understand that Active Health Solutions, LLC and its employees are NOT responsible for any of my belongings that are damaged, stolen, or lost in or around any Active Health Solutions, LLC locations. I acknowledge that I am liable for any damages I may cause to the equipment and/or infrastructure of the facility and agree to reimburse Active Health Solutions, LLC for damages that I cause. I will not allow any non-member to use my key card and gain access to the facility. If I bring a guest it will only be during staffed hours and will be pre-arranged with club faculty. I understand and agree that I am responsible for any claims, damages, or charges made by or caused by any unauthorized members that I allow into any Active Health Solutions, LLC.
